How to Enroll:

Visit the Enrollment Link: Go to this link *and click on the “New Group Enrollment Package” link under the Resources tab.
Complete the Application: Fill out the enrollment application. Make sure to check the dental rider box on page 3.
Submit Your Payment: Attach your first month’s payment.
Mail Your Application: Send the completed application and payment to:
